July 2004-March 2009 New Island Hospital Bethpage New York (Social worker/discharge planner)Member of discharge plan and case management team responsible for screen new Patients, develop psychosocial assessment, interface with Insurance providers, participate in patient clinical review, participate in different management meetings including ethics meeting especially on patients in critical care and cardiac units, link patients with government establishments as needed, counsel HIV patients and facilitate general discharge Plan.
